What information do I need to order a song?

Short answer

The occasion, the name of the person, your relationship to them and a few memories or quirks. Plus mood, musical style and voice. Five minutes of thinking is enough, and the more specific the details, the more personal the song.

Long answer

The form asks for nothing you would have to look up. It is about the occasion, so a birthday, wedding, anniversary, christening or farewell, about the name of the person, about your relationship to them, and about what you would say about them if you had to give a speech.

The difference between a nice song and a genuinely personal one almost always sits in that one free-text field. General sentences like 'she is a warm person' produce general lines. Specific things carry: the nickname only the family uses, the phrase she always says, the holiday that went wrong, the dog, the hobby, the quirk, the job.

Three to five such details are plenty. You do not need to write them out in full sentences and you do not need to worry about rhymes, notes are enough. If nothing comes to mind, the useful question is: what would make everyone at the table laugh or nod if you brought it up? That is what belongs in the field.
